Please call us, apply online or apply in person. Please have the following items with you when you apply:

  1. Child’s Birth Certificate
  2. Immunization Record
  3. Medical Form
  4. Health Insurance ID card
  5. Proof Of Income
    (3 months of most recent pay stubs, public assistance budget letter, income tax form 1040, W-2 form, Social Security Income form (SSI), Temporary Assistance for Needy Families form (TANF) or Foster/Kinship/Custodial Care documents)
  6. Proof Of Address (2 most recent documents such as a rent bill, utility bill, or bank statement)

We now have the ability for you to upload the above documents electronically. Because the documents we ask for have sensitive information, you will need to register for a secure account.

For security purposes, your account will need to be approved by Community Parents, Inc. After you receive an email that you have been approved, you will need to:

  • Login to your secure area
  • Take pictures of your documents
  • Upload them electronically
